Polycystic Ovary syndrome

Saturday, 26 June 2010

Please note that I will not be able to answer any questions concerning PCOS. Kindly consult your doctor.

Polycystic ovary syndrome is our topic today.

It is a common disorder nowadays. And because it is common i decided to bring to you a summarized post about it.

What is polycystic ovary syndrome ?
Poly-: means many
- Cystic: means cystic cavities

It is an endocrinological disorder characterized by multiple small ovarian cysts, signs of androgen excess, anovulation (no ovulation), menstrual disturbances (it ranges from lighter period to no period).

It was first described by Stein and Leventhal in 1935. Polycystic ovary syndrome is also known as the Stein Leventhal syndrome.

What causes Polycystic ovary syndrome ?
Till now, the exact cause is unknown. However several mechanisms and factors are involved in it.

There is a genetic factor involved, which means that polycystic ovary syndrome may run in families especially in first degree relatives. Besides obesity is a common finding in patients having polycystic ovary syndrome.

One of these mechanisms are started by Insulin resistance (the female body become resistant to the action of her own insulin). Insulin resistance causes the body to produce more insulin to compensate the present resistance.
This is called Hyperinsulinemia (high insulin in blood). The high insulin level will eventually lead to more free androgens (testosterone).

At the end, you get more testosterone in the female body and this what brings the symptoms that will be mentioned next.

Symptoms of Polycystic ovary syndrome:
1. Menstrual dysfunction: the woman can present with less periods in the year or cessation of menstruation (amenorrhea).

2. Infertility: due to altered hormonal balance and due to increased testosterone level.

3. Hirsutism, hair growing on the face or acne: these effects are due to increased testosterone.

In some cases, there are no signs of increased testosterone and diagnosis is made by visualization of the polycystic ovary by ultrasound.

The diagnosis is the job of the doctor so we will leave that to him/her.

However there are important advices to any woman having polycystic ovary syndrome:
1. Reduce your weight because this will make your body more responsive to the medications.(i know this is not easy but do your best). You can do that by:
  • Lowering the carbohydrates and increasing proteins in your diet.
  • Walk daily for at least 30 minutes.
  • If possible go to a gym and start exercising.
  • If these measures fail to reduce your weight, your doctor can prescribe Metformin to help you reduce your weight.
2. If you have excess hair, a medication can be prescribed. Besides you can use depilators as a temporary measure for the situation.
3. Be an active individual and don't get used to a sedentary life.
4. Try to increase the fruits and vegetables in your diet.
5. Try to share your daily activities with a friend and be more social.
6. If you are trying to get pregnant, discuss with your doctor having ovulation inducing drugs. These drugs include Clomid, HMG/HCG injections.

Lines of treatment in polycystic ovary syndrome:
There is medication and there is surgery.

Medication will include contraceptive pills , metformin, Clomid, anti-androgen drugs.

The surgery used in polycystic ovary syndrome is known as ovarian drilling done by laparoscopy.
The benefits of medication and surgery varies from a woman to another. It depends on the woman's age and desire for pregnancy.

  1. Its important to always call it PCOS, not PCO. PCO is a symptom that any woman can have at any given moment in their lives, regardless of health. PCOS is the disease and yes, many do exhibit PCO but not all. PCOS does not equal PCO and vise versa. Using PCO in reference to the disease also hinders the job of us educators and strengthens the false idea that PCOS is a reproductive disorder when in reality, its an endocrine disease. Reproductive problems are only the byproduct of the disease.

  2. Hello
I am a 21 years old girl. Last year I went to doctor because of not having my periods for 4 months. She then asked me to do sonography and some other tests. Finally told me that I have PCOD.
Then she asked me to take Harmoni for 6months giving a gap of 1mon after 3 months. i.e 3(medicine)+1(gap)+3(medicine)
Then she again did some tests for androgens and thn it was 280.
So she asked me to take Asmita for another 3 months and after that 3cycles have passed.
The 1st cycle was after exactly 30days(without pill)
next was after 43 days and the recent was after 50days.
Now I am its the 43rd day(i.e. on 22nd sept)
Doctor told me that its normal with me if the cycle is of 40-50 days.

I am so confused about it. Can you please suggest me what to do.

  3. Hello Guddi,
Due to the high androgens level, your periods will come less often. I recommend you follow up with your doctor with the current medications.

i am 22 years old girl. married. i am suffering from Pcod. doctor adviced me to take krimson 35 for three months along with glycomet 250. my weight is normal and i dont experience any other symptoms. every month when i finish the last tab of krimson 35 it takes 7 days to get my periods, is this normal? or within how many days should i get my periods?and what are the chances of getting pregnant after this three months course of tablets. please make me clear about these things.. thank you

    ReplyDelete
  5. Hello,
it is normal to get the period after 7 days from stopping Krimson. In this way, you should be having your period every 28 days.
Your case seems mild as you do not have symptoms, so hopefully these medications will have a good effect after three months

  9. Hello,

Did you do any hormonal tests recently ? The length of time that would be taken for pregnancy to occur depends on the severity of the condition.
I am optimistic because your period became regular and a regular period indicates the occurence of ovulation.

I took a 17ohp test and my progestrone level was 1.24. doc said it is ok. she gave me tab letroz which i have started from my fifth day for five days, and asked me to come on my 11th day. are these good signs to become pregnant? i am so much eager to become pregnant this time itself.

  12. hello

sorry. i dont remember the day on which i took the test.on my 29 th day i believe. i went for the follicular study on 11 th day. my foliicle length was 11 mm. doc said it should be 14mm. they asked me to come again on 13th day. on that day it was 15mm which was alsio not upto the requirment it seems. so doc said chances of getting pregnant is not so bright. she said only one follicle is found. what do you think? can i get pregnant this time? why do this happen even after taking tablets like letroz 2.5mg , folsafe and ovacare.

I've already tried using anti-androgen pill, but I got my period two weeks after the other. So I stopped...
What could you recommend..Thank you...I really need your help...

Concerning your medications, you must consult your doctor for the duration of use.
Your thyroid hormones should be well controlled so that pregnancy can occur.
Did you do a prolactin test ? Please inform me of your previous investigations.

  20. Hello sir,

What is the purpose of duphaston tablet. It helps to concive.

    Thank You

    ReplyDelete
  21. Hello,

Duphaston contains the hormone progesterone. It is used to replace the deficient progesterone in the luteal phase in women having luteal phase defect.
Besides, it can be used to prevent threatened abortion.

  41. There are other options to stimulate your ovaries. The duration of taking Letrozole depends on your doctor's decision.

You should follow-up closely with your doctor to see if Letrozole is the best choice for your case. The follow-up process should include frequent ultrasonography.

  54. Hello,

First of all, PCOS is not related to genital warts. Genital warts is a viral infection.
PCOS can be treated by laparoscopic surgery. However you need to check your thyroid function tests. Hyperthyroidism can be a cause of infertility.

  61. Dear Dr. Ahmad,

My name is Sana and I'm 30 years old. I live in dubai but originally from india. I had visited India a week ago and got diagnosed with PCOS.

    Test results:
Both ovaries are mildly bulky with peripherally arranged multiple small follicles. Right ovary also reveals dominant follicles. Right ovary is 11cc and Left ovary is 11.6

    AMH: 3.99

My doc prescribed Krimson 35. But she did not explain the medication properly. On each question/enquiry, she would get harsh and hyper. I could not visit another doc as I was really tight on my schedule. And I would not want to visit doctors here in Dubai as I've had really bad experiences before with incompetent ones. So I please request you to confirm the following arrangement as i understood from my Doc...

She suggested I take Sysron - N - 5mg - twice a day for 5 days to get periods. After i stop these tablets - I could get periods within 1 to 7 days..is it correct??

Then only after getting period - I start Krimson 35 on the first day of my period and continue for 21 days...is it correct??

Then when i stop, I could again get period within 1 to 7 days when I start the 2nd cycle of Krimson and continue... is it correct??

    My additional doubts are...
    1) Is it normal to not get period within 1-7 days after stopping Krimson 35?? As in what if i get after 2 weeks or 3 weeks..?
    2) What if i dont get my periods after stopping the krimson 35 after 1st cycle itself??? Do I just wait till I get my periods???
    3) What are common side effects of Krimson I should watch out for???

I am into Sales where in I have loads of stress plus I'm going to get married in May, Inshallah. Which is another stress. So in such situation, Would krimson have any added side-effects due to so much stress??? Any suggestion???

    Additional Test results:

    Prolactin : 16.84
    TSH : 5.22
    Free T3 : 2.38
    Free T4 : 1.10
    TSH (ultrasensitive): 6.21

    Does that suggest hypothyroid condition???

She prescribed : Thyrowel for 1 month and asked me to do a TSH again after 1 month with continuing the capsules and send her the result so that accordingly she can advise me....I am taking it since 4 days now, hoping its the right medication.

    Genetic history:

My mom had her uterus removed at the age of 40 as she suffered from excessive bleeding for almost 9 years. I am not sure what this condition is called. She conceived 3 kids in the span of 15 years with no family planning. The reason I am mentioning this is because I am worried with my above medical conditions and disease, plus my genetic history, Would i be able to conceive??? Is it curable, worse or worst??

I'm really sorry to put u through such a long post, But I would be very grateful if you could read it all and reply. I am feeling tad helpless because of my mental, physical and financial situation at this point in time.

    thanks in advance

    Sana

First of all, i would like to thank you for explaining your condition in a neat way. I wish you a happy marriage Mabrook :)
Concerning the doses of Sysron - N and Krimson you are correct. You should have a period when you stop Krimson. Common side effects for Krimson includes nausea, bloating, breast enlargement and tenderness.
Adding stress will not add side effects.

Concerning conception, it is possible because even if you are not ovulating your doctor will prescribe a drug that induces ovulation.

I agree with your doctor's decision concerning the thyroid condition. Thyrowel is a drug that contains vitamin b12, folic acid, iron, zinc and selenium. If the thyroid tests didn't normalize, then this can be due to hypothyroidism.

Feel free to post any questions you have.

    Best Regards,
    Dr. Ahmad

  63. Dear Dr. Ahmad,

Thank you for the prompt reply and your good wishes :)

I have a question: Why did she prescribe thyrowel??? Is it thyroid hormone supplement? What does my thyroid test signify???

    thanks

    Sana

    1. Thyrowel is not thyroid hormone replacement, however it is prescribed in micronutrient deficiencies. Your thyroid tests show that your thyroid isn't producing the normal level of thyroid hormones.

If you are trying to conceive then your doctor may prescribe another class of medication that will help induce ovulation.
It is impossible for me to decide whether you will be able to conceive or not which will be decided by your body's response to medications.

  68. Dear Dr.Ahmad,
Firstly i would like to to tell you that i have been having irregular period since it comes. It started in my 12 and started with 2 to 3 or 4 times a year. Its so irregular but i neglected about it. Only after i was 23,in 2009,I visited OG for the case of itchy in my viginal area. She gave me some viginal pessaries. N then its gone. Also She found out that i was having irregular cycle n suggested me do some tests and ultrasound. Then,i did.All was fine.Consider in another option n took prolactin test. Its too high then normal. She asked me do brain scan to confirm its micro prolactinoma or macro.Fortunately, its micro. She said no harm but it causes irregularity. I had to take bromocriptine to reduce its level,my period came regular again. But not in the 28 days cycles.1 time 1 month.Doesage was adjusted per her option on prolatin test results.Sometimes 2 months taking ,stop, gap n again for 2 months. In the early 2011,after i finished my medication,it became irregular again so visited OG again n she asked me do a lot of tests including TSH and Chest X-ray.I was so much disappointed of doing tests again n again so i let it go. Didnt do
it.My period was so irregular. But in the end of 2011.i got married. We tried for baby nearly a year but didnt occur. By the way,i m from burmer, india origin but now living in singapore. So,I went back to burmer in october 2012,i visited my OG n did a TVS.Its confirmed that i m having PCOS on both ovaries this time. So,without trying on medication as first line, decided to do leproscopic ovarian drilling on both ovaries on the date of 18 December 2012.After the operation,my period showed on 8 jan 2013. Its my last period. Now,i m back in singapore. We decided to get suggestion from fertility specialist here.He gave me dydrogesterone to induce my period n ask me come on the 2nd or 3rd day of my period to do blood test.I really hope of having a baby. The question is ..whats the reason of doing blood test in the 2 or 3rd day of period?Plz,help.
So sorry for putting you through such a long story.Thank u so much for giving your precious time.
    oh' i forgot.i m roopa n in my 27:-)

i m roopa again. My prolactin test were normal. That's why my gynae referred to do Ultrasound n TVS.Also did a lot of tests including LH,FSH.Ovarian drilling was done. Can it b remove cysts in my ovaries. Does it just make only to reduce my testerone level?I really wanna know that cysts r still in my ovaries or not after drilling. Why my period doesnt come regular after its done. I 've been going through such a hard time for getting a baby.I m also having abdominal uneasy n often pain around abdomen n waist.Why?Plz,help...

    1. Hi Roopa,

First of all, thanks for taking your time to explain accurately what you have been through.
I recommend you write your name instead of anonymous for to make it easy for me to follow-up.

The blood test in day 2 or day 3 is for testing FSH and LH levels. This will give an idea about the ovarian reserve and the quality of the eggs produced.
Ovarian drilling is not done for the sake of removing the cysts present however it involves making 4 - 10 punctures in the ovary. In patients having PCOS, the outer thick surface of the ovary may have been producing more testosterone than normal females. When creating punctures in that thick surface, this helps lower dramatically testosterone levels and hence promotes ovulation.

If you are still having a problem with getting pregnant, then your doctor may try medications that stimulate ovulation. Let's see the results of your blood tests.

      Best Regards,
      Dr. Ahmad

    1. Hi Zaan,

PCOS can cause some problems with getting pregnant. According to the severity of your case, your doctor will decide whether to continue on medications or to go for surgery.
You will have to see how your body will respond to medications first. Pregnancy won't happen when you are taking Krimson. If you wish to get pregnant, you should tell your doctor so that further medications will be prescribed.

The surgical option is suitable for those having severe forms of PCOS and for those who don't respond to medications.
I recommend you discuss with your doctor adding a medication that induces ovulation. These medications have shown good results with many patients.

    1. Hi,

To make diagnosis of PCOS, two of the following three criteria should be present:
      - Menstrual irregularity
      - Clinical and/or biochemical signs of hyperandrogenism
      - Polycystic ovaries (by ultrasound)

The sizes of your ovaries are not too big. The duration of treatment will depending on the degree of PCOS.
If you are overweight, reduce your intake of carbohydrates and start practicing sports.

  93. Dear Doctor,
I am 19 years old. studying BTech 3rd year. I stay in hostel. From Dec 2012, I have been suffering from Irregular periods and unexpected weight gain of 7 kgs. I consulted a gynaecologist last july and she suggested some tests and I got tests done. I was diagnosed with Hypothyroidism and PCOD. I was asked to take Krimson 35 for PCOD and Thyronorm and shelcal for hyporthyroidism. I started using Krimson 35 from today. I read some side effects in the manual that comes with the strip. I searched over internet and was wondered that is a oral contraceptive pill. I am a student and is it safe to take an oral contraceptive pill? I am a layman regarding this thing.I have zero knowledge regarding this problem.
Does this problem effect my studies? Does this causes any depression? I am really worried about this. Can you please clarify my doubt. Does taking this medicine increase my fertility issues after my marriage.
How does Krimson 35 sheds weight? Is there any mechanism that it sheds weight?
I would be very thankful to u for clarifying my doubt.

    1. Hi,

Yes it is used as a contraceptive pill in order to regularize your period. It will not affect your fertility. Krimson will not affect your weight. However you should change your diet by reducing your calorie intake and also practicing any form of sport.

  95. Hi Doctor,
I've been reading the Q/A session here which is very informative regarding my questions on the usage of Krimson35. I'm married and have bilateral polycystic ovaries. I'm on my pills for acne (owing to elevated testosterone) and I should say, they are really effective on me. But as for the side effects I've been having so many :) which dreaded me for the past few months. My concern is regarding the light bleeding/spotting that I'm having constantly. I don't use napkins or pantyliners. It is mostly brown (seems like old blood). I read the leaflet which says the 'breakthrough bleeding' is normal in the first few cycles. I've completed 3 and yet have 3 more cycles to complete. During the first set of pills I did not have this bleeding, it all started when I was on my 2nd set of pills. I have also been having some menstrual-like cramps at times, but the bleeding is not heavy. My periods are of normal flow and cramps are less when compared to before I started taking the pills. They are regular and I was regular on periods even before my pills though I had a little late cycle. I have never encountered absent periods any time.
I reported to my gynaecologist when I started seeing this spotting. She asked me to ignore until it is not as heavy as my periods. But, there is no single day that passes without this spotting. How concerned I should be? Can I really ignore it though it is there everyday or is my spotting being abnormally frequent?
Your input would be of great help.

    1. Hi,

I would like to know whether the bleeding appears in the form of some spots or heavier ?

    2. Hi Doctor,
Thank you for the response. The bleeding is like, the mucus that is coated with the brown blood. It is light, not heavy. Initially, it was very light and was not even coming on my panties (I noticed it on my toilet tissues), then it used to stain my panties a bit. But during these spotting days, there has not been a necessity to use a pad.

    3. In my opinion, you shouldn't worry about it.

    4. I'm relieved actually with your response. Thank you Doctor. Can I have one more question regarding the same? Will this medication cause any disturbance to the digestive system other than nausea and indigestion? I have some on/off mild cramping around the right of my bellybutton. Specifically, can you please tell me if there could be disturbance/pain to the liver or gallbladder? I have a mild pain on the right side of my upper abdomen which comes and goes, not persistent. I even had bitter vomitting few weeks back, but fine after that. How serious this could be?

    5. Using birth control pills may increase your risk of gallbladder stones. I recommend you do an abdominal ultrasound to check for the presence of gallbladder stones.

  98. Hi doc. I am 28 years Riya married. We are planning to get pregnant. I always knew I had PCOD at the age of 20 (was under birth control pills Yasmin to regularize my periods). Now, after 2 years of marriage, we are actively trying for baby. My gynae prescribed me Metamorfin for 8 months. And advised me to loose weight my height 5:5. I was weighing 70 Kg. Now I reduced to 61 KG owing to the gym I have joined in August and due to met. I started having Met on June 2013.
First period on 25 July. It was brownish in color. I thought it to be implantation bleeding since we withdraw protection. Tested but negative..

Then again I got brown spotting on 10 August. That lasted for 4 days. Did not fill my pad. But it was there. 7 sep 2013 at office while having green tea I noticed red blood n rushed to loo. This time it was not brown but red. I thought it to be period. But, it stopped altogether. Tested but negative. Urine test came out negative. Called doctor, She asked me to start taking Krimson for 6 months and continue Met.. and asked me to do test at 15 sep. Can you please throw some light on this matter. Dear doctor..

You have been so much kind to all of us.. And also, I have faced no problem in weight loss.. And, I hope it can make me better fertile..Thanku dear doctor Riya Can i take pregnancy test day after tomorrow. If I am pregannt will it will affect my baby since 9 sep i started having krimson 35 for the first time..

    1. Hi,

In my opinion your chances of getting pregnant are good. I do not recommend using birth control pills if you are suspecting pregnancy.
You should do a home pregnancy test whenever you have a late period. You can do a test after tomorrow and if negative confirm it on 15 Sep.

  104. Hello,
I am a 22 year old girl, and suffer from hirsutism. When I consulted the doctor, he said that I have cysts in my ovaries, but they aren't causing any menstrual problems (no irregularity as such, although it seems that I menstruate less than normal, no heavy flow, and mostly for 3 days only.) He has prescribed glycomet and finasteride, to be used for 4 months to check the hirsutism. I was wondering, does this mean I do not have PCOS? What are those cysts then? And how effective are these medicines to help me with hirsutism? Do they have any side effects?

I hope you can help me with my queries.

But i'm thinking that this tables only help to get my cycle

Is this tablets are going to balance my harmons?

of Krimson-35 for one month , i felt that again the hair growth is
increasing as previous, that's why i'm using krimson-35 again without taking my doctors permession

Endoformin will help you loose weight besides to a healthy diet and practicing daily sport, for eg, walking for 30 minutes daily.
If you are thinking of getting pregnant again, try to decide quickly because the quicker the better.
You will have to re-consult your doctor because Krimson is a contraceptive pill. Besides, you may need other medications that will induce ovulation.

    3. It is true that after the age of 35 certain congenital anomalies as Down's syndrome increases in addition to the increased risk of maternal health problems during pregnancy as hypertension and gestational diabetes.
This why you will have to follow-up regularly with your doctor.

Concerning the treatments you have had, I do not think that it will affect your baby as it was from long time ago.

Women normally conceive after doing caesarean section.

With PCOs there is a slightly increased risk of developing uterine cancer.

Common side effects for Krimson includes nausea, bloating, breast enlargement and tenderness.
Weight loss can be difficult in PCOS that's why medications (as metformin) can be used with diet control and lifestyle modification.
Please discuss it with your doctor.

  129. Hello doc!
Firstly thank you for the post, very informative.
I was regularly getting my periods until Feb' 14. I waited for a while and then, On 17th March I consulted my gynec and a scan was done after which she said my ovaries were bulky and prescribed sysron-n for 5 days 2 tabs a day. On Friday I finished the course but still no period! Usually how many days does it take to period after taking this medicine . I have also been advised to loose some weight. I.ve been going to the gym for 6 months now I practise yoga too. I have started eating healthy now. Doc I am 29 and not yet married. Will this have any effect on pregnancy in the future?
    Awaiting your reply.
